 Unify env-var expansion on ${VAR} syntax across config 
Promote _expand_env_vars from mcp/setup.py to oterm.utils.expand_env_vars
and call it from _resolve_api_key. openaiCompatible api_key now accepts
${VAR} and ${VAR:-default} (matching pydantic-ai's MCP convention) and
logs a warning when an env var is missing. Bare $VAR no longer
recognised.
