Migrate from ollama-python to pydantic-ai ...

Replace the direct ollama client with pydantic-ai Agent, enabling
multi-provider support (OpenAI, Anthropic, Google, Mistral, Cohere,
Groq, DeepSeek, Cerebras, Grok, HuggingFace, Bedrock) alongside Ollama.

- Add provider selection to chat edit UI with dynamic model listing
  from provider APIs (with KnownModelName fallback)
- Add per-model capability detection (tools, thinking, vision)
- Migrate tool system from ollama.Tool to pydantic_ai.Tool
- Migrate streaming from ollama client to pydantic-ai run_stream
- Add db migration for provider column and tool format change
- Remove old db upgrade scripts (keep only v0_15_0)
- Update all tests with VCR cassettes for pydantic-ai
