What a triage and release pass had to invent, written down: - pr-queue skill: snapshot open PRs with the fields that decide them (policy-only failures, fork PRs whose tests never ran), group duplicates by linked issue, one verdict per PR, and approval batches. - release skill: a regression gate before proposing a patch, with the downstream smoke, adversarial per-commit review checked against the last tag, fix-on-fix chains as a revert signal, and features held for minors. - CLAUDE.md: premise and blast radius review rules, the downstream consumers and the FastMCP APIs each one calls, and no git stash across worktrees. - dev-docs/known-issues.md: open gaps that are known and not regressions, so reviews stop re-reporting them.
